Public Medical Insurance System

Japan has public medical insurance systems.

Foreign nationals listed on the Basic Resident Register who stay in Japan for purposes other than sightseeing or medical treatment are required to enroll in one of the public medical insurances.
Once you get enrolled in a medical insurance system, you can receive medical care at hospitals at a partial cost.

There are three types of public medical insurance:

  1. Company’s Health Insurance, Mutual Aid Insurance, and Sailor Insurance
  2. Medical Insurance System for the Elderly
  3. National Health Insurance

1 Company’s Health Insurance, Mutual Aid Insurance, and Sailor Insurance

(1) Enrollee

Company’s Health Insurance is an insurance that people who work for companies or business establishments and their families are enrolled in.

(2) Enrollment and withdrawal procedures

The company etc. performs enrollment and withdrawal procedures.

(3) How to pay insurance premiums

The insurance premium is deducted from your salary.

2 National Health Insurance (NHI)

(1) Enrollee

This insurance covers those who are not enrolled in Company’s Health Insurance, etc.

(2) Enrollment and withdrawal procedures

When you enroll in or withdraw from the NHI, you need to submit the notification by yourself.

When you move to Miyazaki City
  • Identification card (* 1)
  • Certificate of move-out from another municipality (only when moving in from another municipality)
When you withdraw from your Company's Health Insurance
  • Identification card
  • Health Insurance Disqualification Notice (* 2) or Certificate of Social Insurance Subscription Period (* 3)
When you are no longer a dependent of your family member enrolled in a health insurance system
At the end of optional continued enrollment in a health insurance system
  • Identification card
  • Health Insurance Disqualification Notice or Health Insurance Insured Card (* 4) with a scheduled date of disqualification for optional continued enrollment entered
When a child was born
  • Identification card

(* 1) Residence Card, Permanent Resident Certificate, passport, driver's license, etc.
(* 2) Issued by the workplace.
(* 3) Issued by the Pension Office.
(* 4) Required for all the persons who are to enroll in NHI

  • NHI Card
When you enroll in Company’s Health Insurance

When you have become a dependent of your family member enrolled in a health insurance system
When the insured person of NHI died
  • NHI Card

(* 5) If Company’s Health Insurance Card has not been issued, a proof that you have enrolled in Company’s Health Insurance.

When your address in the city has been changed
  • NHI Card
When the head of your household or your name has been changed
When your household has been divided or combined with another
If you set a separate address for study
  • Identification card
  • NHI Card
  • Certificate of enrollment (* 6) or student certificate (* 7)
  • If you are a new first-year student and have not yet been issued a certificate of enrollment or student certificate, a letter of acceptance or admission letter
If you have lost your health insurance card or the card is too damaged to use
  • Identification card

(* 6) Must be issued in the relevant year.
(* 7) Must not expire. (A copy is allowed)

(3) How to pay insurance tax

There are two methods for paying insurance tax:
Ordinary payment: Payment through direct debit or with a payment slip
Special payment: Payment by deduction from the public pension

3 What is the Medical Insurance System for the Elderly?

(1) Enrollee

The following people are eligible for enrollment:

  1. All people aged 75 years and over (From the 75th birthday)
  2. Persons aged 65 to 74 with certain disabilities (From the date of certification)

(2) Enrollment method

[1] Those who have reached the age of 75

No enrollment notification is required.

[2] Those who have a certain disability certification

For the enrollment procedure, contact the "National Insurance and Pension Division".
